Review your household and address

Confirm who needs coverage and where each person lives. Gather information for everyone who will be included on the application.

Estimate 2027 household income

Prepare a reasonable annual estimate for each person who files taxes. Include expected changes in employment or income.

Gather your documents

When applicable, have Social Security numbers, immigration documents, tax information, and income details available.

Make a list of your care needs

Write down your doctors, clinics, hospitals, and medications. Check provider networks and coverage for each plan you compare.

Review your current coverage

Read renewal notices and review monthly costs and benefit changes. Do not assume your current plan will stay the same.

Check your Marketplace account

Make sure you can sign in to HealthCare.gov. Use the official site to review and submit your application.

Protect your information: Do not send your Social Security number, identity documents, or medical information by email or social media. Arrange a suitable way to provide documents when needed.
When choosing a plan

Look beyond the monthly premium.

Review the plan’s possible total costs and how it fits your needs: deductible, copayments, medications, provider network, and out-of-pocket maximum. Plan availability and costs depend on your application and the plans offered in your area.

After choosing, follow the instructions from the Marketplace and the insurance company. Coverage does not begin until enrollment is complete and the initial premium is paid when required.

What happens after I submit the form?

We will contact you. Before looking for plan options, you will receive a consent form through MyFMG to review and sign.

Does completing the form enroll me in a plan?

No. The form gathers information so we can help with an application. Coverage requires completing the application, selecting a plan, and following the Marketplace and insurer’s steps.

Should I review my current plan if I want to renew?

Yes. Review the premium, doctors, medications, network, and benefits for the new year. Options and costs may change.

Will my doctor and medications be covered?

That depends on the specific plan and where you live. Confirm the provider network and drug list before selecting a plan.

Can I get help in English or Spanish?

Yes. Contact Roell or Carla by phone or email. The intake form also has a language selector.
